Please refer to either the PDF or MSWord version of the Registration Form.  Either form version may be printed and completed(filled out).  You can use PayPal to pay all fees but the form MUST be mailed to Ken Davis, either by "snail mail" or by "e-mail", along with the completed Banquet Menu selections for each attendee.  This will ensure that all names for Members and Guests are documented and banquet preparations can be made for the Reunion.
